Pathway of Hope - A 5 Point Plan

1. Identify families with potential from within our "safety net" emergency assistance programs. 2.Pair families with a case manager to assess their situation, identify their strengths, and set measurable goals. 3. Connect families with resources and opportunities to help them reach self-sufficiency. 4. Meet regularly with the families and hold them accountable. If they are not demonstrating progress toward their goals, they are asked to leave to make room for another family.Pathway of Hope Regional Coordinator: Krystal Ambrozaitis.
